PROCEDURES FOR AVAILMENT OF THE COLLEGE EDUCATION PROGRAM (CEP)

  FOR THE INITIAL AVAILMENT:  

1.  The Sponsor should bring the following required documents:

2. The above documents should be submitted one (1) month ahead or on the scheduled availment date to our Head Office Claims Department or any of our Branches nearest to the Sponsor.

3. In case of valid absence of the Sponsor (ex. Residence outside the Philippines), the authorized representative should bring a Special Power of Attorney signed by the Sponsor, authenticated abroad by the Philippine Consulate with his/her 2 valid Identification Cards together with all the requirements mentioned above.

4. When claiming the check, the Claimant should sign the Check Voucher and indicate the date of receipt.

FOR THE FINAL/FULL AVAILMENT:

2. When claiming the check, the Claimant should sign the Check Voucher and the Deed of Settlement and Certificate of Release, to be duly notarized at Claimant's own expense, prior to the release of the final check/s.

FOR THE AVAILMENT OF POST-AVAILMENT CASH BENEFIT:  

NOTE: IN CASES WHERE XEROX OR PHOTOCOPIES OF THE REQUIRED DOCUMENTS ARE SUBMITTED, THE ORIGINAL MUST BE PRESENTED FOR AUTHENTICATION.

 

PROCEDURES FOR CLAIMING THE LIFE PLAN DEATH BENEFIT

Designated Beneficiary/ies should:

  1. Secure Claimant's Statement Form from Head Office or any of HPPI Branches -- to be accomplished completely and signed by each beneficiary (except minor/s) -- must be notarized.
  2. If the account is Contestable, also secure Attending Physician's Statement Form from Head Office or any of HPPI Branches -- to be accomplished completely and signed by the regular physician of the planholder and must be notarized.
  3. If with minor beneficiary/ies, also secure Affidavit of Guardianship Form -- to be accomplished completely and signed by the minor's guardian and must also be notarized (if share of the minor does not exceed P50,000) .
  4. Submit the above form/s together with the required documents. (See List of Required Documents).

In cases where xerox or photocopies are submitted, the original must be presented for authentication.

PROCESSING OF CLAIM WILL COMMENCE ONLY UPON SUBMISSION OF COMPLETE DOCUMENTS

IMPORTANT REMINDER: As provided for in the General Provisions of our Contract, all documents required in the filing of claims shall have been completed and submitted to the Company Head Office within one (1) year from the date of death of the planholder. Thereafter, any claim for benefits shall be forever barred.
